Forms for new acupuncture patients
Please complete the New Patient Form before your first acupuncture appointment. Either print it out at home and bring the completed form to your appointment or arrive 15-20 minutes early to complete a New Patient Form in our waiting room.
Note about Financial Policy: Payment must be made by check, credit/debit card or PayPal at time of treatment. No shows and cancellations within 12 hours of scheduled appointment are subject to $20 cancellation fee.
